Both. Tattoos done in one sitting are usually charged a flat rate, which is agreed upon before the tattoo is started. This is what is referred to as charging by the piece. When a tattoo takes multiple sittings, you are charged by the hour. Sleeves, leg pieces, chest panels, and back pieces are all examples of work that is charged by the hour. Estimates on pieces that are worked on hourly vary by the size of the area being covered, imagery, amount of detail, and whether it is done in black and gray or color.
